Terms of Service, Confidentiality & Release Agreement

For all sessions and Services with Lisa J. Smith

Please read the following terms carefully. By scheduling and participating in a session with Lisa J. Smith, you acknowledge that you have read, understood, and agreed to the following Terms of Service. Your participation is entirely voluntary.

1. Age Requirement

Clients must be 18 years of age or older to receive a session.

2. Voluntary Participation

By scheduling, paying for, and initiating contact with Lisa J. Smith, you confirm that you are participating of your own free will and that you accept all terms outlined in this agreement.

3. Prepayment Policy

All sessions must be paid in full prior to the scheduled appointment. Sessions are not confirmed until payment has been received.

4. Appointment Responsibility

It is the client’s responsibility to call Lisa J. Smith at the scheduled time at (248) 593-5227. Lisa does not initiateclient calls.

5. Cancellation Policy

Cancellations must be made at least 24 hours in advance of the scheduled session.

• To cancel or reschedule, call: (248) 593-5227.

• Cancellations made with less than 24-hour notice are non-refundable. See below

6. No-Show Policy

If you do not attend your appointment without prior notice, it will be treated as a no-show.

• No-shows are not eligible for rescheduling or refunds.

7. Refund Policy

Sessions become non-refundable after the first 10 minutes of the scheduled time.

• If you are dissatisfied during the first 10 minutes, you may request a refund.

• After that point, no refunds will be issued.

8. Late Arrival

If you are more than 10 minutes late, your session will be forfeited with no refund.

• Any late minutes will be deducted from your scheduled session time.

9. Scheduling Confirmation

After payment is received, Lisa will contact you within 24 hours to schedule your session time

10. Service, Confidentiality & Release Agreement

By scheduling, making payment for, and initiating contact with Lisa J. Smith, you acknowledge that you have read, understood, and agreed to the following Terms of Service, Confidentiality & Release Agreement. To proceed with your session, you must complete and submit the form below as a record of your informed consent to enter into this agreement with Lisa J. Smith("Smith") regarding my participation in any services, sessions, or readings provided by Lisa J. Smith.

1. Voluntary Participation

I confirm that I am voluntarily requesting a reading/service from Lisa J. Smith and that no guarantees or promises have been made regarding the outcome.

2. Not a Substitute for Professional Services

I understand that these services are not a replacement for medical, psychological, psychiatric, legal, or financial advice or treatment. I agree to seek professional assistance when needed.

3. Personal Responsibility

I accept full and sole responsibility for my mental, emotional, physical, medical, and spiritual well-being before, during, and after the session.

4. Medical and Therapeutic Acknowledgment

I acknowledge that it is my responsibility to assess whether I may require professional support and to seek it independently of any session with Smith.

5. Release of Liability

In consideration of my voluntary participation, I hereby release and discharge Lisa J. Smith, her office, employees, partners, affiliates, and agents from any and all liability, claims, or demands, known or unknown, arising from my participation in any session or service.

6. Waiver of Claims

I agree not to initiate or pursue any legal action, claim, or demand against Lisa J. Smith for any reason related to my session, regardless of outcome or perceived effectiveness.

7. Indemnification

I agree to hold Lisa J. Smith harmless and to indemnify her against any legal claims, damages, costs, or expenses—including attorney’s fees—resulting from my participation in her services.

8. Confidentiality

All personal information and details shared during the session are confidential and will not be disclosed by Lisa J. Smith unless required by law.

9. Understanding & Acceptance

I confirm that I have carefully read this agreement in full. I understand and accept all terms, including the release of liability and waiver of claims. I am entering into this agreement freely and voluntarily.
